Finding and accessing needed health care services can be stressful and overwhelming when you or a loved one require immediate care, or are making ongoing decisions.

To improve overall efficiency and positive patient interactions with regional health services across the province, the Ontario Government is building a connected health care system centred around patients, families and caregivers. These Ontario Health Teams (OHTs), are groups of health-care providers and organizations that are clinically and financially accountable for delivering a full and coordinated continuum of care to their specific geographic population.
