Team
Building a connected health care system centred around patients, families & caregivers.
Ontario Health Teams (OHTs) are designed to help existing health care organizations shape and deliver care that is more connected to patients in their local communities. As voices of the Rainy River District Ontario Health Team (RRDOHT), our local health-care contributors work as one collaborative team, no matter where they offer care.
Since the winter of 2021, the RRDOHT has been building a foundation to bring primary care, acute care, mental health and addictions services, long-term care, home care and community care options closer to home, regardless of what stage of health care you’re currently in.
Our Mission and Vision
Mission
Connected care that is centred around you.
When we say connected, we mean...
- We have connected care pathways
- Information, communication & technology providers are connected
- Our decisions are collaborative and connected to the Mission, Vision and Values
- We have a collective voice advocating for our community members
- We collaborate to eliminate barriers to care
Vision
Exceptional health care. Close to home.
When we say exceptional, we mean care that is:
- Accessible
- Equitable
- Safe
- Timely
- Effective
- Efficient
- Patient Centred
And that we have the capacity to provide the care ( we have the people, time, ability, etc. ).
We are striving to be exceptional in everything we do, now and into the future.
