Diabetes is a chronic condition where the body struggles to store and use energy from food. It's increasingly prevalent in our district and is a top priority for the Rainy River District Ontario Health Team and our partners. We are dedicated to improving diabetes management and outcomes. On this page, you'll find local health care partners providing diabetes education and care, ensuring you receive the support you need close to home.

Atikokan Health
& Community Services
Managed by the Atikokan Family Health Team, this program offers clients a permanent liaison who assists with referrals and medication refills. They work to ensure the best navigation and the most effective treatment and prevention possible.
Diabetes Care Services
- Diabetes education and support for prediabetes, type 1, type 2 and gestational diabetes
- Point of Care A1C (blood) testing
- Annual diabetes screening lab work requisition initiation
- Medication management recommendations
- Insulin education
- Foot screening
- Blood pressure monitoring
- CNIB eye van referrals
- Home visits for diabetes management and follow-up assessments
- Collaboration with physicians, nurse practitioners, nursing, and dieticians through shared EMR and direct messaging
Education & Prevention
- Education and support for pre-diabetes
- Education in the workplace, including daycares and schools for teaching staff and students
Referrals
Can be accessed through self-referral or referral from any care provider via phone, fax, or EMR
Eligibility
Will see patients of any age, however, those under 18 are recommended to have primary diabetes care provided by physicians or paediatric specialists

Fort Frances Family Health Team
The Family Health Team specializes in services for high-risk diabetic patients, and other rostered clients requiring specialty foot-care services.

Giishkaandago'Ikwe
Health Services
Giishkaandago’Ikwe is increasing the knowledge and understanding of healthy lifestyle choices, empowering clients in our Anishinaabe communities to reduce complications associated with diabetes by setting goals in relation to diet, exercise, and medication.
Diabetes Care Services
- One-on-one support to clients includes blood pressure and glucose monitoring clinics, and education as needed to support clients in managing diabetes
- Advanced foot-care services
- Support from Home and Community Care team for ongoing nursing services
Education & Prevention
- Education on healthy living in the Children Services Programs, including access to dietitian and physiotherapist

Accepting referrals from all sources, including self-referral form on website

Gizhewaadiziwin Health
Access Centre
Diabetes educators and experts in diabetes management serving First Nations, Metis and their families. The Gizhewaadiziwin Health Access Centre provides knowledge, skills and support to help people live well with diabetes.
Diabetes Care Services
- Clients receiving primary care at GHAC work closely with physicians and nurse practitioners in the areas of:
- medication recommendation and management
- insulin starts and adjustment
- Blood (A1C) testing and annual diabetes lab screening
- Clients can be seen in Fort Frances or their home community
Education & Prevention
- Group and individual education in:
- blood sugar monitoring
- hypoglycemia prevention and management
- nutrition and smoking cessation
- medications and insulin
- retinopathy screening
- blood pressure monitoring

Accepted from primary care providers in the Rainy River District. Self-referrals also accepted

Riverside Healthcare
Riverside Health Care's community-based diabetes program offers education and care for residents of the Rainy River District while serving as a resource to other local health care professionals.
