Priority Population 24/7 Navigation Supports Implementation Plan

SUMMARY:
With a deep dive into policies, procedures, network capabilities, communications, medical record systems and more, the RRDOHT used surveys to determine how best to approach healthcare navigation in the district for priority populations in mental health, addictions, and diabetic care. The end goal for users is a 24/7 digital navigation system that will streamline the digital patient experience, creating easier access to health care services, including after-hours, and up-to-date information. This Implementation Plan has branched out into multiple projects, including a push for more onboarding of eReferrals in the district. The plan has multiple phases spanning the 2022-23 fiscal year.
